Ongoing innovation; managed risk or reckless abandon?

For every example of a market in which being a first mover has catapulted an idea to mainstream success, there are a dozen where the same actions have created less than stellar responses. 

Indeed in some cases the actions have not just been neutral in effect, but have damaged the brand and reputation of the organisation or individual.
